The Sustainable Health Questionnaire

The Sustainable Health Questionnaire (SHQ) is a 2nd-generation online health assessment questionnaire. The initial concept was developed in the 1990s by Professor William L. Haskell, PhD, of Stanford University, and used in the Cardiovascular Risk Assessment (CRA) tool of the Stanford Heart Network. The CRA was one of the very first online tools now known by the name Health Risk Assessment, or HRA. The CRA was designed to help corporate wellness programs collaborate with physicians in the clinic. The 3rd-generation SHQ, currently in development, will be a broad-scale healthy networking tool with integrated lifestyle data acquisition and management.

Geoffrey E. Moore, MD FACSM

Geoff provides Sustainable Health Systems with a rare breadth of expertise on how to implement lifestyle interventions in medical services, from the clinical model to practice operations and processes to billing and optimizing productivity, providing insight on how we can help improve America's health care system. He has extensive experience as a physician-scientist, an internist and sports medicine physician, a private-practice physician, founder of a hospital-based lifestyle service (where he provided the clinical, business, staffing and billing models), as well as advocacy experience on state and national health care policy. Geoff provides us with the perspective that our tools need to support a stable business model that elegantly uses technology to help enable physicians to engage with patients on lifestyle and health promotion.

In formal training, Dr. Moore completed a residency in internal medicine in 1993 and became board-certified later that year. In addition, he completed 3 years of research fellowship in cardiovascular disease and exercise science at U.T. Southwestern, a 1-year clinical fellowship at McGill University in musculoskeletal / sports medicine, and had a 5-year Clinical Investigator Development Award from the National Institutes of Health. He worked as a research associate at the Stanford Center for Research in Disease Prevention, as an Assistant Professor at the University of Pittsburgh, and is a Fellow of the American College of Sports Medicine, with 40 peer-reviewed research publications. Geoff is senior editor for the 4th edition of a textbook published by the American College of Sports Medicine - ACSM's Exercise Management for Persons with Chronic Diseases and Disabilities. Dr. Moore moved to Ithaca, NY, in 2007 to become Director of Clinical Services at the Cayuga Center for Healthy Living. Since 2003, he has been deeply involved in local, regional and national advocacy efforts through the American College of Sports Medicine and the Medical Society of the State of New York. A runner since 1969, Geoff was a regional standout in high school, ran NCAA Division I track and cross-country at Brown University, was an assistant coach of women's track and cross-country at Brown, competed in the 1984 and 1992 US Olympic Trials, and ran 3 sub-2:20 marathons with a lifetime best of 2:17:41. Having given up competitive sports, he now jogs for fun and likes to cycle, windsurf and cross-country ski.

Stephen Lynch, BS

Stephen H. Lynch has worked in enterprise program development, learning innovation and commercialization leadership roles for over thirty years. His service has included being the U.S. Director of End User Training with IBM’s Catapult subsidiary; Chief Learning Officer with UK- based KMS, a knowledge management software development firm funded by HSBC; a director with Dublin-based EMG building interactive learning programs; and as a founding partner with privately-held I/Tech Services, an early stage online content and curriculum developer supplying programs to the academic marketplace before being acquired in 2005.

Steve additionally served as a vice president and New England General Manager with CESC, an Albany, NY-based technical and professional services subsidiary of Sun Capital Partners. Steve also worked at Boston University as a line manager in the Corporate Education Center designing, developing and implementing mission-critical certified project and process management training programs, management development, critical skills and leadership competency programs via a global network of education affiliates in 50 United States and Canadian marketplaces, including working with the United States Army.

Immediately prior to joining Massachusetts General Hospital, Lynch was Director of Learning Solutions at Toolwire, Inc., a venture funded experiential distance learning platform provider specializing in immersive digital media, virtualization and epistemic gaming content that supports some of the world’s largest global software companies, corporate education providers and institutions of higher education.

Steve is fluently conversational in French and Spanish, in addition to having a passion for lifelong learning, linguistic anthropology, and etymology. Steve and Karen, his partner of 40 years, reside in Norwell, MA, and enjoy spending time being outdoors or on the water whenever fair wind, tide and time permit.

Peter Sepsis, MS MPH

Peter provides strategic planning and market guidance for Sustainable Health Systems, with his passion for primary prevention as well as for consulting with our clients. Peter brings Sustainable Health Systems an ability to evaluate our client's needs and capacities, clarify project roles and tasks, create and communicate reasonable and agreed-upon expectations, all while fostering ability to remain flexible and persevere through difficult challenges. He acquired these skills in his academic and vocational experiences, as well as his avocational activities.

Peter occasionally travels to Ithaca, but is based in Pasadena, CA, where he is a Senior Consultant in HealthWorks and Product Innovation at Kaiser Permanente of Southern California. At Kaiser, he is responsible for consulting with large employer groups to improve their health culture and hence the well being of the emplyee population. Peter also directs the Southern California Center for Healthy Living where he focuses on the development, implementation, and evaluation of educational programs related to physical, social, and psychological health. The Department of Retirement Services in the City of San Jose (CA) has been his client for over 15 years. His previous experience includes design, implementation, and evaluation of cardiovascular disease prevention programs while at the Center for Research and Disease Prevention at Stanford University and the Institute for Health and Aging at the University of California, San Francisco. In addition, Peter developed and implemented stress management programs at Pasadena Psychiatric Medical Group for patients experiencing post traumatic stress disorders. Peter was a mountaineering instructor at Colorado Outward Bound School and has been a lifelong jazz musician, he is currently in a Los Angeles jazz ensemble called TriTone Asylum, and both these experiences contribute to his ability to guide groups forward under sometimes stressful and dynamic circumstances.

Matthew Clark

Matthew Clark is co-founder and Chief Technology Officer for GORGES, providing Sustainable Health Systems with both strategic as well as some of our more highly sophisticated technological guidance. He is involved in our project for automating data aqcuistion from web-enabled devices such as the Pebble Smart Watch, and is one of a dozen Pebble-sponsored app developers. He has worked on Pebble apps for eBay, iControl, Time Warner and PayPal. He knows many programming languages and frameworks, including C++, .NET, PHP, Java-JSP, and QlikView, and has developed solutions using SQL Server, Oracle, and MySQL databases.

After beginning his career at the David Sarnoff Research Center working for RCA and General Electric, Matt worked for several startup companies in New York City and Montreal, consulting for AutoDesk, SPSS, and Data Description prior to founding GORGES. Matt is also a published author, a patented inventor, and part-time volunteer educator at a local school introducing logic and robotics to elementary school children. When not hacking or running marathons, Matt can be found applying his engineering talents building legos with his children.

 

Ted Caldwell

Ted was the primary architect of our Sustainable Health Questionnaire, and provides Sustainable Health Systems with insightful approaches to application development.

Ted is an Application Developer and Senior Software Architect for GORGES, where his work encompasses all aspects of web application development, including requirements analysis, database and software design, programming, project management, and system administration. With over 20 years of software development experience, Ted enjoys working closely with GORGES clients to understand their software needs and create practical, cost-effective technology solutions. His technical skills include systems analysis, software architecture, user interface design and many programming tools: PHP, Javascript, Ruby, Java, Yii, Ruby on Rails, AngularJS, Javascript visualization libraries: Raphael JS, HighCharts, Go JS, as well as mobile application development for iOS and Android.

Prior to joining GORGES, Ted was Computational Linguist and Director of Engineering at CoGenTex, where he led development of several innovative applications based on natural language generation technology. He was the principal developer of a tool for generating textual product recommendations, used by prominent e-tailers including Staples and Target, and holds a U.S. patent related to this work. He also served as Principal Investigator on an NSF-funded project to develop Chart Explainer, a tool that generates natural-language descriptions of charts and graphs, currently licensed to the U.S. Department of Justice.

 

Sean Kennedy

Sean is a designer for GORGES, and provides design services for Sustainable Health Systems. He is skilled at user interface and graphic design, and created the elegant Sustainable Health Systems web interface. He has expertise with the latest web styling tools and techniques, such as CSS3 and XHTML template development.
