Well-Being Toolkits

Our new system organizes health ecology data in a relational database. The items come from the National Institutes of Health project on Grid Enabled Measures. The design of our patient's report page allows health care teams to:

• Be more aware of patients who have disparities,

• Align patient needs to local resources by health plan/covered benefits, and

• Analyze population health risks against health outcomes.

Simply collecting the data on population health risk factors and ecological disparities is not enough. Our system is designed to take the next step and facilitate your local solutions. Domains that we now support include:

• General well-being

• Depression screening (PHQ-9)

• Physical functioning

• Alcohol use (AUDIT)
